To submit a claim, please provide the following documents at your nearest branch or email them to: claims@ljloveevents.co.za
- Certified copy of the Death Certificate
- Certified copy of the Deceased’s ID
- BI 1663 Form (Notice of Death)
- Certified copy of the Main Member’s or Beneficiary’s ID
- Police Report (for unnatural or accidental causes of death)
- Claimant’s ID
Please ensure that all premium payments are up to date at the time of the claim. We reserve the right to request additional documents if needed to properly assess the claim
Claims under this Scheme will not be honored if the event leading to the claim is directly or indirectly caused by, or attributable to, any of the following:
Participation by the Principal Insured and any other insured individuals in:
- Acts of war (declared or not), military actions, riots, insurrections, civil commotion, usurpation of power, martial law, terrorism, or the use of nuclear, chemical, or biological weapons, devices, or agents.
- Disease, epidemics, or pandemics.
Any intentional acts committed by the Principal Insured or other insured persons that violate the law. This includes actions involving a minor child where the parent or legal guardian knowingly allows participation in illegal activities.
Self-inflicted injuries or illnesses, regardless of intent, including voluntary exposure to known risks or dangers. Claims arising from injuries or diseases caused partly by the actions or omissions of the insured, alongside actions or omissions from other parties or contributory factors, will also be excluded.
Taking or consuming any drug, medicine, sedative, or poison, whether accidentally or intentionally, unless it is prescribed by a licensed medical practitioner who is not the Insured.
